The Fairfield Diabetes Education Center provides outpatient information, support and teaching for adults and children diagnosed with diabetes. This includes persons diagnosed with Type I, Type II, Pre-diabetes, and gestational diabetes. The Center delivers diabetes education by an experienced team, including a Registered Nurse/Certified Diabetes Educator and a Registered Dietitian. The Center assures high-quality education to help develop and improve patient self-care skills essential to diabetes management and is recognized by the American Diabetes Association. Family members are encouraged to participate. If you would like to become a client of the Diabetes Education Center, we require a referral from a physician. Once your referral is received, we will contact you and schedule your appointment.
